Reporting an Incident

At San Francisco State University, a campus rooted in social justice and inclusion, all employees are expected to act professionally and treat others with civility and dignity. Behavior that is insensitive, intolerant, or discriminatory has no place in our community.

There are no mandatory reporting requirements for behavior that falls under OCC. Many concerns can be resolved informally, and the following steps are recommended:

  • Speak directly with the individual involved, if you are comfortable and it's appropriate.
  • Contact a supervisor or administrator if direct communication is unsuccessful or not feasible.
  • Submit an OCC report if informal efforts have been exhausted or additional support is needed.

     

If you are a student, faculty member, staff member, or campus visitor who has witnessed or experienced any of the behaviors below, you are encouraged to Submit Incident or by using the QR code below.

Examples of Other Conducts of Concern include:

  • Targeted behavior based on protected status (such as age, disability, gender identity, race, religion, sexual orientation, etc.) that does not meet the legal threshold of being “severe,” “pervasive,” or “persistent” under CSU policy or federal/state law.
  • Materially disruptive conduct that impacts the learning, living, or working environment, even if it may not warrant disciplinary action due to protections under free speech laws.
  • Abusive or unprofessional behavior as defined by California Government Code §12950.1(h)(2) or the Education Code §89535(b), which may result in disciplinary action.
