Public Service Loan Forgiveness (PSLF) - Employment Certification

Program Information

The PSLF Program was established by Congress in 2007 to allow eligible borrowers to qualify for forgiveness of the remaining balance of their William D. Ford Federal Direct Loan after serving full-time at a public service organization for at least 10 years and making 120 qualifying payments.

Employment Certification

Payroll Services will complete Sections 3 and 4 to verify that employment is with a public service organization. Employment Certification forms must be submitted to our office. If you are submitting via email, please ensure that you do not include your Social Security Number (SSN) on the form.
 

It is the responsibility of the PSLF applicant to submit the application and all required paperwork to the U.S. Department of Education to qualify for the PSLF Program. The U.S. Department of Education, not CSU, reviews the applications and determines eligibility for the PSLF Program."

  • Employer Name: California State University, San Francisco
  • Employer's EIN/FEIN: 94-6001347

Submitting your form:

  1. Use the PSLF Help Tool to submit electronically to hrwww@sfsu.edu  OR
  2. Download the form from the appropriate website. Complete the form on your computer without the SSN and save it as a PDF file.  (You may need to "print" to Adobe PDF or Microsoft Print to PDF.)
    • Email the PDF to: hrwww@sfsu.edu
    • Subject: PSLF Employment Certification_Your Name_University ID #
    • Attach the form

          OR

  1. Upload the PDF by submitting an Employment Verification Service Request
  2. Description: PSLF Employment Certification_Your Name_University ID #
  3. Attach/upload the form

Your form will be processed within 3-5 business days.

 

PSLF Return Process:

If you used the PSLF Help Tool to submit your form electronically, it will be completed and submitted directly to Federal Student Aid. If you submitted your form via email request, the completed form will be returned to you through email. For those who submitted a form through a Service Request, your completed form will be uploaded to the Service Request. Employees who submitted a paper form will receive an email notification when the form is ready for pick-up.

Important: After receiving the completed PSLF form from Payroll Services you must add your SSN and signature before submitting it to the requesting agency.
