Change in Employment (Appointment) Status

An employee’s change in employment status may result in changes in benefits eligibility, coverage, and premium costs.

Examples of a change of employment status include:

  • a change from a non-benefits eligible appointment to a benefits-eligible appointment
  • part-time (below 50%) to full-time (50% or above) employment change

Eligibility Information

To be eligible for SF State University's benefits you must

  • be an employee who is paid by San Francisco State University
  • work 50% or more of the normal full-time work schedule and
  • have an eligible appointment.

If you were not covered under an SF State health plan because you did not previously meet eligibility requirements, you can enroll in one of SF State's health plans within 31 days of the date of your appointment change. Use the enrollment/change form below.

You may enroll in the SF State Dependent Care FSA or SF State Health Care FSA within 31 days of the date of your appointment change. Domestic Partners are not eligible for this benefit.

Program

A term life insurance and limited accidental death and dismemberment program which includes an accelerated death benefit for terminal illness.

Monthly Premium

Currently, no premium rate is charged to employees for CSU provided basic life and/or basic accidental death and dismemberment.

Eligibility

Employees are eligible if appointed half-time or more for a period of more than six (6) months.

Lecturers and Coaches appointed for a minimum of one semester or two consecutive quarters at .4 timebase or greater are eligible for benefits. Coaches (Non-academic calendar year) and Lectures (12-month appointments) are eligible for benefits if appointed half-time or more for a period of more than six (6) months. Exclusions: Faculty appointed solely to teach summer sessions, extension or intersession are not eligible for benefits.

Employees appointed on an hourly, intermittent basis do not qualify for benefits.

Enrollment

All eligible employees will be automatically enrolled. Coverage will be effective the first of the month following eligibility.
